What resolution is the photo?

300 DPI, the standard for printing. The value is saved inside the file, which upload forms and photo shops often check.

Can I print several copies on one sheet?

Yes. Print sheet (4 × 6 in) and Print sheet (A4) fit as many copies as possible, with light cut lines.

How do I upload it to an online form?

Download the JPG. If the form also has a file size limit, like 50 KB, use Compress Image to KB or MB next.
